If you are an employee or member whose coverage terminated under an ODS group medical benefit plan, you may be eligible to enroll in an ODS Portability Plan.
All of your dependents may obtain coverage under the ODS Portability Plan provided that your dependents were covered as dependents under your prior plan. Dependents include your spouse and dependent children.

You are eligible for portability coverage under this agreement if you meet the following requirements:
Spouse’s Eligibility
Your spouse is eligible for coverage if he or she is under 65 years of age and is not covered under another health benefit plan.
Children’s Eligibility
Your unmarried children are eligible if they are under age 23 and are not covered under another health benefit plan. A child is also eligible, up to age 23, if a court or administrative order requires you to provide health insurance.
You must choose the plan below that is consistent with the plan you had with your former employer.
You can set up automatic payment for your coverage by submitting the Electronic Funds Transfer form.
